Medication
When a child’s physician prescribes medication to be taken during school hours, state law requires that the physician state the dosage and related information on the Administration of Medication form, which is available in all school offices. Parents are required to sign a release allowing school personnel to dispense medication.
In order for any medication to be dispensed at school, an Administration of Medication form signed by both the physician and parent must be on file in the school office. A new Administration of Medicine form is required each school year.
